Indications/Uses
Metastatic colorectal cancer & adjuvant treatment of stage III (Dukes C) colon cancer in combination w/ fluorouracil & folinic acid.
Dosage/Direction for Use
85 mg/m2 by IV infusion over 2-6 hr, dissolved in 250-500 mL of glucose 5%. May repeated at 2-wk interval & reduced according to tolerance. Adjuvant therapy May be given for 12 cycles. Reduce to 75 mg/m2 after persistent neurotoxicity or recovery from severe adverse effects. Metastatic colorectal cancer Reduce to 65 mg/m2 after persistent neurotoxicity or recovery from severe adverse effects. Always administer before fluoropyrimidines.
Special Precautions
Neurological exam at regular intervals during treatment & reduced dose if symptoms are prolonged or severe. Perform regular blood counts during treatment & courses should not be repeated until blood counts have recovered. Not to be given to patients w/ preexisting sensory neuropathies or myelosuppression, nor those w/ severe renal impairment. Monitor renal function & toxicity carefully in more moderate degrees of renal impairment. Use appropriate contraceptive measures during treatment & for 6 mth (for men) & 4 mth (for women) after stopping treatment.
Adverse Reactions
Raised liver enzyme values. Neurotoxicity. Peripheral neuropathy; pain, functional impairment & loss of tendon reflexes. Pulmonary fibrosis. Extravasation can cause local pain & inflammation; severe complications, including necrosis.
Drug Interactions
Increased plasma conc of fluorouracil. Decreased clearance w/ nephrotoxic drugs.
